I HEREBY ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THE AGREEMENT ABOVE (INITIAL
The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water,
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. A complete bathroom, with wash bowl, tub or shower, toilet stool, hot and cold running
water, . floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all 1994 UBC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until all
the required items are completed and a final inspection made.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE ABOVE
ITEMS HAVE BEEN COMPLETED.
****Cannot occupy or use dwelling until a Certificate of Occupancy (C.O.) is issued.
Occupancy or use of dwelling without a C.O. will be considered an illegal occupancy and may be
grounds for vacating premises until above conditions are met.

CONDMONS OF APPROVAL
1) This well shall be used in such a way as to cause no material injury to existing water rights. The Issuance of the
permit does not assure the applicant that no injury will occur to another vested water right or preclude another
owner of a vested water right from seeking relief in a civil court action,
2) The construction of this well shall be in compliance with the Water Well Construction Flutes 2 OCR 402-2, unless
approval of a variance has been granted by the State Board of Examiners of Water Well Construction and Purnp
Installation Contractors in accordance with Rule 19.
3) Approved pursuant to CPS 37-90-137(2) for the expansion of use of an existing well. permit no. 42570.F
" (canceled), appropriating ground water tributary to the Roaring Fork River, as an alternate polnt of diversion to the
Basalt Conduit, on the condition that the well shall be operated only when the Basalt Water Conservancy District's
substitute water supply plan, approved by the State Engineer, is in effect and when a water allotment contract
between the well owner and the Basalt Water Conservancy District for the release of replacement water from Rued
Reservoir is in effect, or under an approved plan for augmentation. BWCD contract #3.3.5194.
4) The use of ground water from this well Is limited to ordinary household purposes inside three (3) single family
dwellings (two single family dwellings and a guest unit), and the irrigation of not more than 10,000 square feet of
home gardens and lawns, all located on lots A & 8, Larsh Minor Subdivision, Garfield County. All use of ihls well
will be curtailed unless the water allotment contract or a plan for augmentation is in effect.
5) The maximum pumping rate shall not exceed 15 GPM.
6) The average annual amount of ground water to be appropriated shall not exceed one (1.6) acre-feet (521,360
gallons).
7) A totalizing flow meter must be installed on this wail and maintained in good working order. Permanent records
of all diversions must be maintained by the well owner (recorded at least annually) and submitted to the Division
Engineer upon request,
8) The well must be located not more than 200 feet from the location specified on this permit, and not less than 600
feet from any existing well owned by others,
9) The owner shall mark the well In a conspicuous place with well permit number(s), name of the aquifer, and court
case number(s) as appropriate. The owner shall take necessary moans and precautions to preserve these

ALTERATIONS

MOVED BUILDINGS
In order to understand the scope of the work intended under a permit application and expedite
the issuance of a permit it is important that complete information be provided. Adequate and
complete information will prevent delays in the plan review process. Reviewing a plan and the
discovery that required information has not been provided by the applicant may result in the
delay of the permit issuance and m proceeding with building construction. The owner or
contractor may be required to provide this information before the plan review may proceed. This
causes delays because other plans that are m line for review may be given attention before the
new information may be reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and provide adequate information to facilitate a plan review. If you do not, it may
be helpful to obtain a book titled "Dwelling Construction under the Uniform Building
Code". This book is available to you through this department at our cost. Also, please
consider using a design professional for assistance in your design and a construction
professional for construction of your project.
To provide for a more understandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are requested to review the following check list prior
to and during design. Applicants are require to indicate appropriately and to submit the
completed check list at time of application for a permit.
For the placement of a manufactured home, questions numbers 1-4 and 19-28 are the only
questions that need to be addressed:
1. Is a site plan included that indicates the distances of the proposed building or addition to property
lines, other buildings, set back easements and utility easements?

the distances to property lines, wells (on subject property and adjacent properties), streams or
