RE: Inspection of footing formwork and concrete reinforcement
At about 10:30 A.M. on November 23, 2016, Steve Millard, P.E. and Al Rakowski of
Rakowski -Millard Structural Design met Bruce Upton at the subject site. The purpose of the
site visit was to verify that the footing formwork dimensions and the steel reinforcing for
the concrete were in conformance with the structural design drawings prepared by
Rakowski -Millard Structural Design, dated 11/18/16, and in compliance with IBC 2012 and
AC1 318-08 minimum code requirements.
Millard and Rakowski confirmed visually and with limited measurements that the footing
formwork dimensions and the steel reinforcing appeared to conform to the above
mentioned design drawings and code required minimums. No physical testing was
performed.
