RE: Review of OWTS for 1800 Midtand Avenue, Simpson Residence: #Sept-02-17-4583
Dear Andy:
The purpose of this correspondence is to confirm the design and construction of the OWTS for the
Simpson Residence located at 1800 Midland Avenue in Glenwood Springs.
On Tnesclay, May 16,2017 the percolation rate was determined to be 28 minutes per inch. This
was congnrent with the assumed soils infiltration rates and this confirmed that the OV/TS could
be constructed as designed. Review of the profile hole showed that there was no bedrock or free
water table within the leaching area and subsoils were as described in the subsoils study per HP-
Kumar dated July 12,2016.
On Tuesday, May 23,2017 and on Thursday, May 2.5,2017,Mountain Cross Engineering, Inc.
iirspected the OWTS construction. In the professional opinion of Chris Hale, as a þrofessional
.rrjin..,. licensed in the State of Colorado, #35964, and a representative of Mountain Cross
Engineering, Inc, it appeared to be installed per the plans prepared by Mountain Cross Engineering,
Inc.
