The onsite wastewater treatment system of reference was inspected on September 22, 2016. The
system was found to conform with the plans and specifications set forth in the engineered
sewage disposal system design for the aforementioned property prepared on June 30, 2016 with
the following note: an equivalent Arc 36 Biodiffusers (34"x60") were used for a total of 36 unit
(180'). Swimming pools, ponds, or other water features were not included in the submitted or
approved design. Copies of the engineered plan may be obtained from the undersigned design
engineer.
