Dear Larry,
This is to confirm our conversation with you and Don Owens when we visited your property at
1539 County Road 130 In Glenwood last week.
You must submit plans for approval, along with a permit application, for the new office space that
you are going to build.
You must submit a permit application, accompanied by "as built" drawings, signed by a Colorado
licensed Architect or Engineer, for the apartment on the second floor of the building that was built
without a permit.

Dear Mr. Owens;
As per our telephone conversation of 14 January, 1 found that the structure located at 1539
County Road 130 has a number of multi - occupancy U.B.C. deficiencies, specifically the
inadequacy of separations between the residence and other units. Due to the high hazard
occupancy that adjoins the residential occupancy at this location, we feel this property has a
significant potential to cause further injury or death.
At this time our department, in conjunction with KN Energy, has identified and mitigated the
immediate carbon monoxide issue that alerted us to the other construction and code deficiencies.

January 14, 1999
Ron Bullock- Fireman for the city of Glenwood,called to tell me of an incident that happened the
previous Tuesday 1 -6 -99. They were called to 1539 County Road 130 (Donegan)Where tight
people were treated for Carbon Monoxide. He ask me if I knew about it, I did not.
That afternoon I went there and found a commercial building housing an Auto Repair Shop, and a
shop that makes Marble Countcr Tops, with an apartment on the second floor, where the people
lived that got gassed.
I talked to a Jason Wiethoff, the owner and leaser of the Auto Repair shop, He told me the
Firemen investigations found a faulty installed space heater vent system, was the cause, letting the
fumes go into the apartment, on the second floor.
When I arrived about 5p.m. the venting had been repaired, which I observed from the outside .1
was not able to get either the apt. or the Marble Top Co.
Jason told me he has been leasing that space for over ten years, and when he first moved in, the
owner, Larry Martin, lived in the Apt. up stairs.
1 have looked for the Building Permits and inspections of this address, and the inclose copies are
all I can find.
