Colorado - PAE #16028 To Whom It May Concern: Please be advised that Pattillo Associates Engineers was involved with certain aspects of the structural repairs that were accomplished for the subject residence in 2016 and 2017. Our scope of work included consultation pertaining to the methods and extent of foundation underpinning, periodic observation of compaction grouting operations, design of new, concrete wall "infill" panels above the basement window heads, and measurement of structure benchmark elevations for future monitoring purposes. With regard to the compaction grouting underpinning repairs that were done to stabilize the structure from further settlement, we observed the operations of Hayward Baker on several occasions and reviewed grouting logs. A total of 204 cubic yards of grout was injected in 59 locations around the perimeter of the building, at an average depth of 26.5 feet. The work was verified to comply with the Grouting Plans prepared by J.R. Harris & Company, dated June 17, 2016. Our office also prepared a design to improve the structural span capacity of the east basement wall by infilling the area above the two window openings with reinforced concrete. A copy of the drawing dated September 22, 2016, is attached to this report. We inspected the reinforcing and formwork prior to concrete placement and hereby confirm that the construction complied with the design. Lastly,_we conducted an elevation survey on May 11, 2017, to establish the relative elevations of 59 marked points within the house and 8 points in the detached garage. This survey is intended to serve as a benchmark reference to verify the stability of the buildings in the future. The locations of the points are shown on the attached "Monitoring Plan" drawing, and the calculated elevations are shown on the attached "Relative Elevations Summary" chart. The level measurements were made using a calibrated manometer, considered accurate to plus or minus one-tenth inch. The survey can be repeated by request at any time in the future to detect any post -stabilization vertical movement that may or may not occur in the interim period between surveys. ......
