Dear Mr. Crossland;
Enclosed please find the Scheduled Field Maintenance Report for the Onsite Wastewater Treatment System
currently serving the above referenced location. The system was inspected on April 14, 2015. The system, as
designed, employs advanced treatment technology in the form of a recirculating packed bed textile filter
treatment system (AdvanTex AX20 treatment unit).
This type of system requires an ongoing operation and maintenance agreement with an authorized service
provider. I have included an invoice for the current 2015 maintenance agreement. The next inspection is
scheduled for October, 2015. At that time, it is recommended that you have the septic tank pumped. Other
recommendations are included on the Report. The current maintenance agreement with CCI Engineering
expires on March 13, 2016.
