To Whom it May Concern:
The purpose of this letter is to address the construction of the Onsite Wastewater Treatment System for
the Derkash Residence located on Lot 51 of the Westbank Ranch PUD, Filing IV in Garfield County,
CO. (south of Glenwood Springs). Boundaries Unlimited Inc. (BUI) performed an inspections of the
installed system on in September 2016 and July 2017. The installation included a 2,000 -gallon
concrete septic tank, a dosing pump system, a cleanout near the house, 80 ARC -36 gravelless
chamber units in a 4x20 (12'x100') bed configuration, flush valves and inspection ports.
The system appeared to have been installed in conformance with the intent of the design. The
contractor was directed to backfill with onsite select soils. I have enclosed photos of the construction.

Garfield County Building Department,
This letter is to inform the building department that I plan to use the space above my garage at 1574
Huebinger Dr. for my personal use and it will not be a rental unit.
