November 20, 1989 TO: FROM: ALL PLAN PARTICIPANTS BOARD OF DIRECTORS RE: IMPORTANT PLAN MODIFICATIONS, EFFECTIVE JANUARY 1, 1990 Your county has always been concerned with providing you and your family quality health care. Our goal is to continue to provide the most competitive, comprehensive benefits possible in a cost·effective and affordable manner. In order to accomplish our goals and to provide you with fast and efficient claims payment, the following Plan Modifications will become effective January 1, 1990. * UTILIZATION REVIEW PROGRAM Currently, in order to avoid a hospital dedu<.:tible, you call Parkside's Patient Services Program. You are not required to comply with any pan of the program once the call is made. This Parkside program has been changed for all admissions occurring on or after January 1, 1990. It will become a Pre·Certification/Certification program with the following requirements: 1. Your or the patient's attending physician must call Parkside to certify the need for hospital admission. For elective (planned) admissions this call must be done in advance. Emergency admissions must be reported within 48 hours. Each request for admission will be reviewed to evaluate the necessity and appropriateness of admission and a pre--Oetermined period of confinement will be established. 2. Failure to comply with the Pre·Certification/Certification requirements will result in a reduction in benefits. The penalty for non-compliance is a 20% coinsurance reduction to a maximum penalty of $1,500 per admission. * DISCRETIONARY BENEFITS It is no longer necessary to contact Parkside for outpatient chemotherapy and radiation treatment. Member/Non-Member Pharmacy -Plan pays 70% of cost for Bmnd name or Generic Drugs. Dispensing Limitations -30 days supply or 100 tablets or capsules, whichever is greater. In the near future, you will receive a prescription drug card from Scrip Card which you must present to the pharmacy when purchasing a prescription. All prescriptions filled on or after January 1, 1990 must be submitted to Scrip Card. National Benefit Administrators will not process any prescription drug claims incurred (purchased) after December 31, 1989. PSYCHIATRIC/SUBSTANCE ABUSE TREATMENT Inpatient benefits for mental and nervous conditions or substance abuse will be payab'e at QO% and will not increase to 100% payment. Outpatient psychiatric therapy and substance abuse therapy will continue to be paid at 50% up to a new calendar year maximum benefit of $1,500. * EXTENSION OF BENEFITS The Board has decided to eliminate the Extension of Benefits provision from the Plan.
