CONDITIONS:
1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage
Disposal Systems Chapter 25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and
building requirements. Connection to or use with any dwelling or structures not approved by the Building and
Zoning office shall automatically be a violation or a requirement of the permit and cause for both legal action
and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which
involves a knowing and material variation from the terms or specifications contained in the application of
permit commits a Class 1, Petty Offense ($500.00 fine - 6 months in jail or both).

ALL SERVICE septic, LLC performed an installation observation on November 25th 2008 for the
subject property.
The installation included the existing 1250 -gallon, two-compartment precast concrete septic tank
with the addition of a new Biotube pump vault followed by 3 rows of 20 `Quick -4' chambers, for a
total of 60 chambers, equaling 593 SF of drain field.
L5 -inch perforated laterals were suspended within each row of chambers for the entire length of each
chamber row. The laterals are pressurized and have '/a -inch holes on 4.5 -foot centers. Drain holes
were drilled to drain the laterals after each pump dose.
The OWS was installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials ALL SERVICE septic, LLC should be notified if changes
are made to the OWS.
LIMITS:
The observations were based on information submitted. If conditions are different from conditions
described in this letter, ALL SERVICE septic, LLC should be notified. All additional OWS
construction must be according to the county regulations. The installer should have documented and
demonstrated knowledge of the requirements and regulations of the county in which they are
working.
