25, Article 10 C.R.S. 1973, Revised 1984.
2, This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.

ALL SERVICE septic, LLC performed an installation observation on July 14th 2009 for the subject
property.
The installation included a 1250 -gallon, two-compartment concrete septic tank with an effluent :filter
on the outlet, followed by a 500 gallon concrete dosing tank with a pump. One mounded pressurized
bed with imported sand filter material was installed. The bed had 2 feet of sand filter material and 4
rows of 9 `Quick -4' chambers. The basal area of the imported sand filter material was 20 feet by 40
feet, equaling an infiltrative area of 800 SF. Effluent is distributed with 1.5 -inch pressurized laterals
and a 1.5 -inch manifold.
The OWS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWS.
LIMITS:
The observations were based on information submitted. If conditions are different from conditions
described in this letter, ALL SERVICE septic, LLC should be notified. All additional OWS
construction must be according to the county regulations. The installer should have documented and
demonstrated knowledge of the requirements and regulations of the county in which they are
working.

As requested, ALL SERVICE septic, LLC has prepared an alternative septic tank design report for
the subject residence. The residence and septic system are currently under construction.
A 1250 -gallon, two-compartment septic tank, with an inlet tee and a BiotubeTM pump vault may be
installed at the subject property. A pump curve and tank detail are included in report. PVC and
concrete risers and secure lids must be installed on the septic tank, to bring access to grade. If the
septic tank is placed in a driveway concrete `traffic rated' lids and risers must be used. A standard P-
30 OrencoTM pump package and control panel is appropriate to serve septic system.
The septic tank should be pumped every two to three years, and the tees inspected. If required, an
effluent filter should be placed on the outlet of the septic tank. The effluent filter should be cleaned as
needed.
LIMITS:
The design is based on information submitted. If conditions on site are different from conditions
assumed in report, ALL SERVICE septic, LLC should be notified. All OWS construction must be
according to the county regulations. Requirements not specified in this report must follow county
regulations. The installer should have documented and demonstrated knowledge of the requirements
and regulations of the county in which they are working. This design for the subject property, noted
above, is valid only in conjunction with the approved County Health Department permit.
