1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation ora
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class 1, Patty Offense ($500.00 fine — 6
months in jail or both).

Dear Gentleman:
Pursuant to County Regulations, this letter provides documentation that the new Onsite Wastewater
System (OWS) system recently installed is in general compliance with the permitted design. Sopris
Engineering has performed site visits to inspect and document the as built conditions of the constructed
system. An inspection of the existing septic system revealed that the existing field and tank was in poor
condition and needed to be replaced. The system was not failed however capacity was limited. The
replacement onsite wastewater system constructed on site was specified and built to serve the needs of a 2
bedroom single family residence at 5097 C.R. 214, New Castle CO, situated in Section 6, T 6 S, R 91 W
of the 6'11P.M. We have coordinated our efforts with the owner and contractor (Murr Excavation) that
constructed the system. The system was inspected prior to backfilling, after all installations were
completed. The as -built conditions and installation of the new OWS components is in compliance with the
permitted design specifications for the system.
The septic tank, distribution pipes, and absorption field installations are in accordance with Garfield
County Regulations and the design criteria provided to the owner and contractor by Sopris Engineering in
July, 2007. New 4" SDR -35 sewer pipe and cleanout was installed from the existing waste line out of the
structure to the new septic tank. A new Copland 1000 gallon dual compartment concrete septic tank was
installed with 4" tee baffles and an effluent filter. The absorption field was constructed with a total of 26
Quick 4 leaching chambers in a trench configuration with (2) 55' long trenches containing 13 chambers
each. The chambers were backfilled with screened native material. Each trench was installed level.
Inspection ports were installed on each end chamber. The chambers were installed in suitable soils
consistent across the field. The field has individual distribution lines from a tuf lite I-IDPE distribution
box, connected to the head of each trench. The distribution box was installed level on compacted ground
and has risers with a lid at the surface. The minimum setback distances have been maintained. The 2
trenches have a minimum separation distance of 6 feet and are installed approximately 2-3 feet deep
relative to the existing site grades. Additional observations of the soils within the field were performed
during construction indicating that the soils are consistent with the geotechnical evaluation and observation
performed by Sopris Engineering prior to construction.

Subsurface soil investigations and Percolation tests were performed by Sopris Engineering, LLC. Five
percolation test holes were prepared approximately 36 to 48 inches deep to calculate percolation rates and
to confirm the onsite soils are suitable for conventional absorption systems. A profile pit was excavation to
observe the soil in the area of the field replacement. The average rate was determined to be 23 MPI and
was used for design. The fastest measured rate was 13 MPI and the slowest rate was 40 MPI. The soils
below 1/2 foot of topsoil consist of sandy, silty clay loam with scattered cobbles to the 8 foot depth
explored. No free water was encountered in the exploration pit. Groundwater levels are expected to be
below a minimum of 20 feet from the existing surface grades. The soils are suitable for conventional
shallow absorption trenches.
OWS Design Criteria

OWS Operation and Maintenance
The owner shall periodically inspect the OWS, perform any maintenance required and periodically pump
the septic tank as necessary to ensure that the system is in good operating condition and performing as
designed. The tank, sanitary tees and effluent filter should be visually inspected bi-annually for clogging
debris, damage or Leaks. In general, for a properly utilized system, septic tanks should be pumped and
inspected every 2 - 4 years. The effluent filter should be cleaned every six months and at the time of
pumping. Absorption fields should be maintained with suitable vegetative cover and kept free of root
invasive plants. Positive surface drainage away from the absorption field should be maintained.
