•CONDITIONS:
1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is vahd only for connection to structures which have fully complied with County zoning and building requirements. Con·
nect1on to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained 1n the application of permit commits a Class I, Petty Offense ($500.00 fine - 6
months in jail or both).

Dear Mr. Sande:
As requested by Ron Sande; Hepworth -Pawlak Geol«hnical, Inc., performed
peroolation testing at the subject site. The work was done in accordance wilh our
agreement for Professional Services to you, dated June 6, 2003.
Two profiles pits Uld five: pen:olation test boles~ reporteclly excavated on June 9,
2003 by the client at the locatious sl&own oa Fil-1. The subsoils exposed in Profile Pits 1
and 2 conslste.d of about 6 inc:bcs of topsoil overlyina approximately 5 to 7~ feet of silty,
sandy clay, overlyillgsaudy, silty gravel to Ille pit depths of 10 and 8 Ket, respectively.
Water was observed in Pit 1 at 81h feet and soils above the water wc111 slightly moist to
moist No free water was observed in Pit 2 to the depth excavated, 8 feet Percolation
test holes were hand dug In the bottom of shallow backhoe pits and soaked with water on
June 10, 2003.
Percolation lesting wu condudtd on June 11, 20m, by a representative of Hepworth -
Pawlak Geoteclulical, Inc. The percolatioi;i tat results are &nmmuized on Table I. The
mes varied from 8 to 20 minotes per inch. The overall average of die five percolation
tests was about 1' miuutcs per Inch. Due to poteatial for high groundwater in the area,
we recommend a professional civil engineer desip the septic disposal system.
