This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con- nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a requirement of the permit and cause for both legal action and revocation of the permit. 3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which involves a knowing and material vanation from the terms or spec1f1cations contained 1n the application of permit commits a Class I, Petty Offense ($500.00 fine - 6 months 1n jail or both). Sopris Engineering inspected the excavatiOlll 1111d the installation of system components at various times during construction prior to final backfill. The system constructed oa site was designed and built to serve the needs of the newly constructed 3-bedroom house. Sopris Engineering performed visits to inspect and document the as built conditions of the constructed system. We have coordinated our efforts with the system installer. The as built conditions and installation of the new OWS components is in compliance with the pennitted design speci.fications. for the system. Tbe System was installed Jn accordance with Garfield County Regulations; the design presented in the Sopris Enpneering Repon and the design drawing dated April 21, 2004. A 1250-pllon dual compartment concrete septic tan.k was installed with aPP">Priate sanitary tees and an effluent filter in the outlet tee. The absorption field was constructed with a total of 20 standard leaching chambers In a trench configuration with 2 trenches with 10 c:halllbem each. The chambers were baddilled with native soil. &ch trench was installed level. Inspection ports were installad on each comet end chamber. The chambers were installed in suitable !!Olis consistent across the field. Additional observations of the soils within the field were pedonned during construction indicating that the soils are consistent with the descript{on in the geotechnical repon. No pund water was encountered to a depth of 10 feet. The field has indMdual distribution lines to each trench ft'om a concrete distribution box. The distrib11tion box was installed level on compacted ground 1111d is marked with a buried piece of rebar. The construction WRll completed utilizing specified materials installed acconling to design. The minimum setback distances have been maintained. OWS One!'!!tion and M•inJepam All components of the engineered OWS shall be inspected on a regular basis and be properly maintained. The re establishment of vegetative cover over the field and responsibility for inspection and maintenance of the system will remain with the Lot Owner or the owners representative. The owner shall periodically inspcct the OWS and to perfonn all maintenance and repairs as necessary to ensure that the system is in goo~ operating condition and performing as designed. The OWS should require minimal maintenana:. Several factors influencing th.e need for maintenance Include: actual wastewater flows versus design flows, the volume of kitchen/domestic waste (excluding human waste and toilet paper), excessive household cllemicals and other toxic liquids. The tank, sanitary tees and effluent filter should be visually inspected bi-annually for cloggin& debris, damage, leab, or other potential problems.
