1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs,alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine — 6
months in jail or both).

Subject: Percolation Testing, Proposed Septic Disposal System, 0037 County
Road 115 (Red Canyon Road), Garfield County, Colorado.
Dear Mr. Preston:
As requested, Hepworth - Pawlak Geotechnical, Inc., performed percolation testing at
the subject site. The work was done in accordance with our agreement for professional
services to Weatherall Company, Inc., dated April 28, 2004.
We understand that due to area constraints, a seepage pit is being planned for septic
disposal. A long pit had been excavated off of' the northwest corner of the existing
building by the client prior to our arrival on-site. The pit had been terraced with a
percolation test holes excavated at each of three levels at about the locations shown on
Figure 1. The subsoils exposed in the pit consisted of about 4 inches of asphalt
overlying silty sandy gravel with cobbles and boulders to the maximum depth of 14
feet. No free water was observed in the pit and the soils were slightly moist to moist.
Percolation test holes had been soaked by the client prior to our arrival.
Percolation testing was conducted on May 13, 2004, by a representative of Hepworth -
Pawlak Geotechnical, Inc. The percolation test results are summarized on Table 1.
The percolation test results indicate an infiltration rate between 17 and 40 minutes per
inch with an average rate of 32 minutes per inch.

The following letter is to confirm the engineer's acceptance of the designed septic system of the
Weatherall Company on Red Canyon Road (County Road 115). The design included a 1,000
gallon septic tank and drywell. The septic tank, drywell, cleanout, and vent pipe have been
installed. The concrete encasement has been changed and approved to flowfill. At time of
inspection, the effluent filter was not installed, but per phone conversation with client, it has now
been installed.
With the above noted changes, the system has been completed with the intent of the original design
