contractor was notified on a daily basis of our test results and observations.

As requested, a representative of GROUND visited the project to perform construction
observations and materials testing. We observed the construction of the CMU walls at gridlines
A.0 -A.3/2.9-3.3. Two Rivers Masonry was constructing the walls at 16' to 18' high. We observed
the reinforcing steel for specified bar sizing, spacing tolerances, lap splices, and cell clearances.
We also observed the placement of block, bond beams, lintels, ladder placement, mortar
thickness, grout and the overall condition of the wall. The CMU walls and the reinforcing steel
appeared to be constructed and placed in general accordance with the project plans and site
specifications. We cast strength specimens on the mortar for future compressive strength
testing, the results will be forwarded upon completion.
We notified Terry with CMC of all the results of our observations.

the reinforcing steel for specified bar sizing, spacing tolerances, lap splices, and cell clearances.
We also observed the placement of block, bond beams, lintels, ladder placement, mortar
thickness, grout and the overall condition of the wall. We obtained a temperature reading on the
mortar of 51 degrees and on the grout of 60 degrees. We also performed a slump test on the
grout supplied by United/37010744 of 8'/2".The CMU walls and the reinforcing steel appeared to
be constructed and placed in general accordance with the project plans and site specifications.
We cast strength specimens for the mortar, grout and prisms for future compressive strength
testing. The results will be forwarded upon completion.

As requested, a representative of GROUND arrived at the project site to observe welded
connections of joist, beams and angle at the Roof Level and bolted connections of beams and
columns.
The following is a summary of our observations conducted on this date:
1) Observed welded connections of joist to beam and joist to CMU embeds at the Low
Roof between Gridlines A -G/1-3, A.7 -F/3-4 and A -A.7/3.2-4 and found them to be in
general compliance per Details 1 and 6/S2.2 of the project plans and specifications.
2) Observed welded connections of L3x3x1/4 continuous to joist and L3x3x1/4
continuous to beam at the Low Roof between Gridlines A -F/3-4, B.5 -G/1-3 and found
them to be in general compliance per Detail 1/S2.2 of the project plans.
3) Observed welded connections of beams to CMU embeds at the Low Roof Level
between Gridlines A -F/3-4. GROUND observed that the beam to CMU embed welds
was:
A) Welded on both sides of the beam flange to CMU embed with a minimum 3/16"
fillet weld at 2" minimum in length.
B) Welded with one overhead 3/16" fillet in front of CMU embed.
C) No welds at beam to CMU embed. GROUND is requesting written clarification
from the Engineer of Record for this connection, since there is no Weld Detail in
the project plans. Discrepancy Item #1.
4) Observed bolted connections of beam to columns and beam -to -beam at the Low
Roof between Gridlines A -F/3-4 and found them to be in general compliance with the
project plans and specifications.

5) GROUND observed missing 3/4"x5" EXP bolts at L3x3x1/4 continuous at Gridlines C -
F/4, F/3-4 and E/1-3. This will be listed as Discrepancy Item #2.
6) GROUND observed beam W18x35 to HSS 4x4x1/4 column was welded, not bolted
at Gridline C.9/3.1 per the project plans. This will be listed as Discrepancy Item #3.
7) GROUND is requesting Welding Procedure Specifications (WPS's) and Welder
Qualification Test Records (WQTRs) for AWS D1.1 and D1.3 from Patriot Steel.
This will be listed as Discrepancy Item #4.

As requested, a representative of GROUND visited the project to perform construction
observations and materials testing. We observed the reinforcing steel placed by Two Rivers
Masonry for the AHRT building walls at gridlines A.0 -B.0/1.0-2.0. The reinforcing steel was
observed for specified bar sizing, spacing tolerances, lap splices, and form clearances. Based
on our observations it appears that the reinforcing steel was placed in general accordance with
the project plans and specifications.
We observed the construction of the CMU walls at gridlines A.0 -B.0/1.0-2.0, Two Rivers was
constructing walls at elevation 0' to 4' 8" on walls at gridlines A.0 -B.0/1.0-1.5 and on the south
half A.0 -B.0/1.5-2.0, A.0 to 2.0 at 4' 8" to 8' 8".
We observed the placement of block, bond beams, lintels, ladder placement, mortar, grout and
the overall condition of the wall. We obtained a temperature reading on the mortar of 58
degrees and on the grout of 70 degrees. We also performed a slump test on the grout, 9" with
mix ID# 37010744 supplied by United. The CMU walls appeared to be constructed in general
accordance with the project plans and site specifications.
Terry with CMC was notified of all the results of our observations.
Prisms were cast. They are located at the northwest corner of the tented area, upper level.

We also performed a slump test on the grout, 10" with mix ID# 37010744 supplied by United.
The CMU walls were constructed in general accordance with the project plans and site
specifications.
A mortar sample was obtained from approximate gridlines A.0/1.5 for future compressive
strength testing.

4". We observed the placement of CMU mortar, United Mix ID #37010145 grout, bond beams,
reinforcing steel bar size, spacing, clearance, lap splices, ladder placement and the overall
condition of the wall. Based on today's observations the CMU walls appear to be in general
compliance with site specifications.
We obtained a sample of the grout and performed slump and temperature tests. Our results are
as follows: slump 10', temperature 65 degrees.

We observed Two Rivers Masonry begin placing United Mix ID #37010145 grout for masonry
walls on the northeastern building at gridlines A.0 -B.0/1.5-2.0 from 12'8" to 17'4" at
approximately 12:30PM. We obtained a sample of grout and performed slump and temperature
tests. Our results are as follows, slump: 9'/2", temperature: 58 degrees.

welds/tek screws at the roof deck attachment, welded connections of gusset to beam flanges,
bolted connections of beams and columns and re -observe outstanding Discrepancy List items.
The following is a summary of the observations and re -observations conducted on this date:
1) Observed arc spot-welds/tek screws for the roof deck attachment between Gridlines
A -G/1-4 and found them to be in non-compliance due to missing spot welds and
missing tek screws at side laps per the project plans and specifications.
Discrepancy Item #5.
2) Observed welded connections of gusset to beam flange at the High Roof between
Gridlines C -D/3-3.1 and E -E.7/3-3.1 and found them to be in general compliance per
RFI #38 dated January 25, 2010.
3) Observed bolted connections of beams and columns at the High Roof between
Gridlines C -D/3-3.1 and E -E.7/3-3.1 and found them to be in general compliance
with the project plans and specifications.
4) Re -observed Discrepancy Item #3, W18x35 to HSS 4x4x1/4 was welded in lieu of
bolting. This item is now in general compliance per the email dated January 21,
2010, from Ken Martin with Zeiler -Pennock, Inc.

As requested, a representative of GROUND visited the project to perform construction
observations and materials testing.
We observed moisture contents well above optimum in the top 6" to 12" of material as well as
standing water in multiple areas. We utilized a shovel to remove the top 6" of oversaturated
material in the traffic area and observed approximately 18" of frost beneath the mud. In the
landscape area (free of traffic) we observed 9" to 11" of frost with 4" of mud on top of the frost.
Three samples were obtained at 30' north and 30' west of gridlines A.0/1.0, 25' north of gridlines
C.0/2.0, 65' north and 50' west of A.0/1.0. After laboratory analysis, it was determined that the
moisture contents were as follows:
♦ 30' N, 30' W of Gridlines A.0/1.0
♦ 25' N, 0' W of Gridlines C.0/2.0
♦ 65' N, 50' W of Gridlines A.0/1.0
18.4%
20.1%
16.1%
Based on these test results and the typical optimum moisture contents for on-site soils
analyzed, it appears that upon thawing of the subgrade materials, approximately the top 12-18"
of material will likely be above recommended moisture contents for acceptable performance for
subgrade materials.

We observed Advanced Concrete place concrete under and around the base of the steel
support posts at gridlines B.0/3.0. Based on our observations it appeared the concrete was
placed in a manner that forced the concrete underneath the support post plate. This was unable
to be verified though.
The concrete was supplied by United/35510705, approximately 1 cubic yard. This concrete was
excess from Garfield Human Services. It was placed at the Sherriff's Annex at approximately
11:15 AM. Cylinders were cast at Garfield Human Services as well as slump, air and unit
weight.
