Carbondale, CO 81623 (Legal: Section: 28 Township: 7 Range: 88 Subdivision: COOPERTON
TWNSTE. Block: 13 Lot: 19 THRU: - Lot: 21 Parcel: 239328313008) to serve as my representative
throughout the planning, permit, and inspection process for a new septic system on my
property located at 1234 County Road 106, Carbondale, CO 81623. I hereby give Andy Braudis
permission to sign all legal documents pertaining the septic system design, permit, and
inspection.
