DTRECTOR DETERMTNATTON - Mountain Shadow Place Amended Final Plat (FPAA-03-18-86241
Dear Ms. Schwerin and Mr. Rasmussen,
This letter is provided to you, as the authorized representatives for Garfield County File FPAA-O3-
Lg-g624,perta¡nang to an Amended Final Plat to correct the 'gap' created by a surveying error on
Lot 3 of the Mountain Shadow place Subdivision. This letter is notification that your request to
amend the final plat has been reviewed by the Community Development Department and the
Director hereby issues a determination conditionally approving the request. The conditions of
approval are as follows:
!. All representations of the Applicant contained in the Application submittals shall be
conditions of approval unless specifically amended or modified by the conditions
conta¡ned herein.
Z. The proposed plat shall be updated and edited as follows prior to submittal for final
review bY the CountY:
a. The plat shalt include a note indicating the purpoSe of the amended final plat.
b. The plat title shall be revised to read "Amended Final Plat of Lot 3 of Mountain
Shadow Place Minor Subdivision"'
c. The original6.94''gap' that is being corrected by this amended final plat shall be
shown and labeled.
3. The plat shall be subject to final review and approval by County Staff, including Garfield
County Attorney's Office and Garfield County Surveyor, prior to submittal of the Mylar
plat for final execution, BOCC signature and recording'

4. The Applicant has 90 days within which to satisfy conditions of approval and provide the
following documentation for BOCC signature and recordation:
a. A plat mylar with signed Certificates that include Dedication, Ownership, Title,
Taxes, Applicant's Surveyor, Lienholders and any mortgagees.
b. Recordingfees.
Notice of this decision will be provided to the Board of County Commissioners who has the ability
to 'call-up'the application for review at a public hearing. You will be notified of the Board decision
once it is received or upon expiration of the 10 day notice period (May 29th, 2018).
