Herrell 2206 Midland Avenue Glenwood Springs, CO 81601 (970) 379-0464 Garfield County Community Services To Whom It May Concern: October 24, 2018 I am Robert Herrell, co-owner with Jeanne Herrell and George Sharer of the property located at 2206 Midland Avenue, Glenwood Springs, CO, 81601. We are a single family residence as Jeanne is my mother and George is my brother. We do not rent any portion of the home. I am addressing the attachments required in the permit application that I picked up in your office regarding replacement of an existing septic tank that is failing. Our septic tank is 47 years old and has a broken baffle, erosion in the soil surrounding the tank, and deterioration of the concrete access hole in the top of the tank. We are also replacing the clay outflow pipes from the house to the new septic tank and then out to the leech field. Our leech field is still wholly functional and has been tested by the contractor that we are hiring to replace the septic tank. Since we are not replacing the leech field I am assuming that we don't need an engineer, or a percolation test, or a system design blueprint, or the required minimum OWTS. As a result, I have not attached these items to my application. I also think that the inspection is not required, but your department is certainly more knowledgeable than I am! We are replacing an existing 1,000 gallon septic tank with a 1,500 gallon septic tank for a 5 bedroom house. I believe that this meets current regulations as related to me by Mr. Matt Piffer, our contractor. have attached a "site plan," but as you can see, I am not much of a draftsman. I estimated the distance from the house to the tank, and the distance from the house to the property line, and the distance to the nearest city sewage line, as well as the distance down to the river. I am assuming that since this was all correct when the house was built that it should be all right now. Finally, time is of the essence as we are experiencing a near total septic tank fail and the clay pipes are deteriorating (and blocked on a regular basis). Please let me know as soon as possible if I need to provide you with any additional information and I will comply immediately. I appreciate your time and assistance in this matter. Sincerely, 1 Robert M. Herrell
