Our observations and recommendations are presented in this report. The services are supplemental to our agreement for professional services to Ridge Runner Construction dated March 5, 2018. H-P/Kumar previously designed the OWTS for the residence as submitted in our report dated September 4, 2018, Project No. 18-7-187.02. The system was designed for three bedrooms. The OWTS was installed under Garfield County Permit Number SEPT -05-18-5240 by Brent Lough. We performed our As -Built evaluation on September 17 and 20, 2018. During the two visits, the OWTS components observed included: sewer line, cleanouts, septic tank, effluent line, distribution box and soil treatment area (STA) and the locations of these components. The installation and locations of the system components were verified as shown on Figure 1. Pictures of the OWTS components made during our site visits are also included with this report. At the time of our design, the location of the water service line was unknown. During installation, the STA was rearranged to maintain the required minimum 25 -foot setback from the water line Infiltrator Arc 36 chambers were used instead of Infiltrator Quick 4 Plus Standard chambers. The number of chambers was reduced to 36 total due to the larger area of the Arc 36 chambers for a total soil treatment area of 540 square feet (design size was 528 square feet). Ridge Runner Construction November 6, 2018 Page 2 The effluent line was encased in 6" diameter C900 pipe for 10 feet on either side of crossing of the water line as discussed with Brent Lough and shown in a picture provided us, see attached picture log. The septic tank size was increased to 1,250 gallons from 1,000 gallon size recommended in our design. The distribution box was changed to a Tuf-Tite 7 -hole distribution box to accommodate the increased number of rows of chambers. The noted changes from our design are acceptable to us. It is our opinion that the OWTS was installed in general conformance with the above referenced design. The observations and recommendations presented above are based on our site observations and our experience in the area. We make no warranty either express or implied. Alterations to the system or improper backfill and site grading could affect the operation and effectiveness of the OWTS.
