The onsite wastewater treatment system of reference was inspected on November 14,2018. The
system was found to conform with the plans and specifications set forth in the engineered
sewage disposal system design for the aforementioned property prepared on July 20,2018.
Swimming pools, ponds, or other water features were not included in the submitted or approved
design. Setback of OWTS components from property lines and easements are based on
information provided by owner. Copies of the engineered plan rnay be obtained from the
undersigned design engineer.
