The Parcel ID of the property is 2123-242-00-017. You, the property owner, installed the system. The system installation included a poly Infiltrator® 1000 -gallon, two-compartment septic tank with an effluent filter on the outlet tee. This filter must be cleaned annually, or as needed. Risers bring the manhole lids to grade for access. If more than one riser is added to the septic tank, an extension handle must be installed on the effluent filter. Effluent gravity flows to a manifolded chamber bed consisting of four rows of 11 'ARC' ADS chambers for a total of 660 square feet of infiltrative area. Inspection ports were added to each corner of the bed and may be covered with sprinkler boxes for access. Movies were provided during installation, but we were unable to convert these to photographs. Photographs below were taken during our April 9, 2019 visit. At the time of our inspection, it appeared the tank had settled slightly causing the inlet pipe to be kinked. This pipe was excavated and replaced with proper fall. A ranch hydrant was observed near the area STA. Because the hydrant is frost -free, there is an opening in the line for drainage. We recommend this hydrant be relocated at least 25 -feet from the STA to eliminate the possibility of contamination. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The quality of the installation is dependent of the expertise of the installer, soil type, and weather conditions. Please call with questions. Sincerely, ALL SERVICE septic, LLC Carla Ostberg, MPH, REHS Reviewed By: Sewer line excavated after tank settled. Appears to be only a single -sweep clean out rather than a double -sweep.
