MEMORANDUM

Our office originally inspected the noted residence on Nov. 05, 2018. The subject
residence is a two story wood fr'¿imed structure over a crawl space with an attached
vehicular garage. The residential portion of the home measures, in plan, approxinately
35'-0' east-to-west by 47'-0" north-to-south. The home was originally constructbd with
continuous concrete foundation walls and continuous concrete footings. The crawl space
framing included a total of three (3) structural steel beams which clear span eas-t-to-west
asross the crawl space. Those steel beams are supported on pteel columns in the crawl
space.
Our office issued construction documents on November 09, 2018 to address the
stabilization of the foundation walls. Those documents specified the installation of thirty
sevar (37) hydraulically driven steel piles to be installed along the foundation walls. The
construction documents specified the location of each pier and the required "Safe Working
Load" that each pier is to support. Those documents also specified and detailed a total of
four (4) steel brackets to attach the four (4) most heavily loaded steel columns to the
concrete foundation walls.
Foundation Repair of Western Colorado installed the thirty seven (37) steel piers
beginning on May 14,2019 and continuing through May 28, 2019. I performed a visual
inspection of the installed piers on May 28, 2019, and a second inspection on May 29,
2019 to observe installation of the noted steel brackets. They subsequently hydraulically
loaded the piers and lifted and releveled the center portion ofthe home. Our office
coordinated with Foundation Repair of rilestern Colorado during the construction phase.
Subsequent to the foundation repair Foundation Repair of Western Colorado provided a
'Pier Summary" and "Pier Analysis."

Our office reviewed those dooumonts. The Pier Summary includes the driven
depth of each pier and the amount of lift at cach pier. The &iven depths indic¡te the
depth at which the piers encountered the underlying granular bearing soils. The driven
depths u/ere very consistent: they vuied ftom approximately 20 feet to 28 feet.
The liftq as documented, wore zuccessful, and wero as anticþated: the li:ft heights varied
from ll8'to 1".
It is my professional opinion that the foundation repairs have been zuccessfirlly
complaod by Foundation Repair of Western Colorado in accordance with the
construction documents is.sued by our office.
