On Tuesday 06/11/2019 we are going to start re -roofing 15.0 squares of Tamko Heritage Asphalt
Shingles 30 yr. Color mountain slate, Flashing D style drip edge installing all around roof ,
Installing dry in WR. Grace select 3'x all roof eaves and valleys. Installing roofnado storm tread
system 30 yr. Installing new roof jacks as needed. Remove old fascia board on eave and disposal,
install new fascia board (painted

Contract Terms
Mr. M Seamless Gutters, LLC hereby certifies that all roof replacement work performed under the scope
of this contract will be performed in accordance with the requirements set forth by the National Roofing
Contractors' Association (NRCA) and the manufacturers of eaoh oomponent used.
Mr. M Seamless Gutters, LLC shall guarantee the installation of roofing materials including, but not limited
to decking, shingles, ridge vents, and flashing to be in accordance with manufacturers' specifications and
completely watertight for a pei iod of 5 years.
This guarantee includes replacement and reinstallation of any failed or faulty roofing components, but
does not extend to or imply liability for damage to interior building components or contents.
Subject to the guarantee listed above, Mr. M Seamless Gutters, LLC shall, at their sole cost and expense,
make full replacement or repair of the work performed pursuant to the terms of this residential roofing
contract. This includes repair of shingles, flashing, felts, and ridges.
Mr. M Seamless Gutters, LLC agrees to respond to any notification of roofing leak, failure, or fault within
72 hours of notification by Richard Splain.
This roofing guarantee does not cover damage caused by natural causes, such as wind, hail, or lightning.
Likewise, it does not cover damage by the homeowner, additional contractors, or any individual not
employed by Mr. M Seamless Gutters, LLC. Damage caused by these sources shall be the sole
responsibility of Richard Splain to remedy.
This document shall be the sole agreement between the Parties for the services described herein. No
other promises, arrangements, or guarantees not specifically outlined here are valid.
Richard Splain agrees to pay the full amount indicated in this contract within 5 days of notification of
project completion by Mr. M Seamless Gutters, LLC. Failure to pay the full amount by this deadline shall
constitute a breach of contract. Richard Splain may be made by cash, check, or credit card.

Mr. M Seamless Gutters, LLC shall be solely responsible for the safety of their employees, suppliers, and
subcontractors throughout the project. This includes ensuring that all workers follow OSHA job safety
standards at all times.
Mr. M Seamless Gutters, LLC agrees to indemnify and hold Richard Splain harniless against any
damages or liabilities stemming from the scope of this residential roofing contract.
Any disputes related to this contract shall be resolved through a neutral arbitrator. This contract shall be
governed by the laws of Colorado in the event of a dispute, the findings of the neutral arbitrator shall be
considered final and unappealable.
Acceptance
By signing below, the Parties agree to enter into this residential roofing contract as of the date on the first
page of this contract.
