Thiç wett shall be used in such a way as to cause no materiat inJury to existing water r¡ghtt, The issuance of this perffiit does
not ensure that no injury wilt occur to another vested water right or preclude anûther owner of a vested water right from
seeking relief in a civit court action.
The construction of this wetl shatt be in complìance with the Water Wet[ Construction Rules 7. CCR 4t2-2, untess approval of a
variance has been granted by the State Board of Examinerr of Water Wett Constructlon and Pump ln:tallation Contractors in
accçrdance with Rute 18.
Approved pursuänt to CRS 37"92-ó0?{3XbXl} for uses as descrfbed in CRS 37-92-607(1Xf}. Use of this wetl is limited to
monitoring water lsvels and/cr water quatity sampting. This wett is known as Aspen Poto Partners Monitoring/Observation Well
no. Z.
This wett must be equipped wíth a locking cap or seat to prevent well contamination sr posslble ha¡ards a! an open well, The
wetl must be kept capped and locked at all times except during sampting or measuring.
Records of rvater [evel measurements and water quality anatyres shall be mâintâined by the wetl owner and submitted to the
Division of Water Resources upon request.
Upon conclusion of the manitoring progräm the well owner shalt plug thir we[[ fn accordance with Rul,e 1ó of the Water Welt
Construction Rules, A Wett Abandonment Report must be completed and submitted to the Division of Water Resources within 60
days of ptugging.
The cwner shall mark the we{[ in a conspicuous {ocation with the wetl permit number and name of aquifer as appropriate, and
¡halL take necersãry meanr and precautionr to preserve these markings.
This welt must be constructed by or under the supervision of a licensed wetl drilter or other authorized individuat accord'ing to
the Water Wett Construction Rutes, lf non-standard construction is anticipated, a variance request must be submitted in
accordance with Rule 18 and approved prior to wetl construction"
A Wetl Construction and Yietd Estirnate RepÕrt (Form 6W5-31), inctuding tithûlogic log must b€ submitted by the individuat
authorized tû construct the wel{, Fsr non-standard coñstruction, the report must inctude an ¿s-buitt drawing showing detailg

Dear Mr. Marrs:
As requested, Hepworth-Pawlak Geotechnical, Inc. installed monitoring wells at the
zubject site. The wells were installed in accordance with our agreement for professional
services to Noble Landscape Design Studio dated August 13,20A7. The data obtained
and subsurface conditions encountered are presented in this report.
Eight monitoring well$ were inst¿lled at the approximate locations s¡own on Figure l.
The logs ofthe borings are presented on Figure 2, Thezubsoils encountered, below about
one foot of topsoil, mainly consist of silty sandy gravel with cobbles. A sandy siþ clay
layer was observed in Borings 1, 3 and I overlying the gravel. Free water was observed in
Borings 1,2, 5,6,7 and I at the time of drilling and the upper soils were slightly moist to
moist. We understand that watffi level monitoring will be condusted by others.

CCINDITIONS OF APPROVAL
This wett shatt be used in such a way as to cause no material injury to existing water rights. The issuance of this permit does
not ensure that no injury witt occur to another vested water right or preclude another owner of a vested water right from
seeking retief in a civil court action.
The construction of this wett shalt be in compliance with the Water Well Construction Rutes 2 CCR 402-2, untess approval of a
variance has been granted by the State Board of Examiners of Water Wett Construction and Pump lnstaltation Contractors in
accordance with Rute 18.
Approved pursuant to CRS 37-92-602(3r(b)(l) for uses as described in CRS 37-92-602(1Xf ). Use of this we[[ is limited to
monitoring water levets and/or water quatity sampting. This wett is known as Aspen Poto Partners Monitoring/Observation Wel[
no.1.
This wett must be equipped with a tocking cap or seat to prevent wett contamination or possibte hazards as an open wett. The
wetl must be kept. capped and locked at alt times except during sampting or measuring.
Records of water tevel measurements and water quatity anatyses shalt be maintained by the wett owner and submitted to the
Division of Water Resources upon request.
Upon conclusion of the monitoring program the wett owner shatt ptug this wett in accordance with Rute 16 of the Water Well
Construction Rutes. A Wett Abandonment Report must be compteted and submitted to the Division of Water Resources within 60
days of ptugging.
The owner shatl mark the welt in a conspicuous tocation with the wetl permit number and name of aquifer as appropriate, and
shatt take necessary means and precautions to preserve these markings.
This wett must be constructed by or under the supervision of a licensed wetl drilter or other authorized individuat according to
the Water We[[ Construction Rutes. lf non-standard construction is anticipated, a variance request must be submitted in
accordance with Rute 18 and approved prior to wetl construction.
A Wett Construction and Yield Estimate Report (Form GW5-31), inctuding tithotogic log must be submitted by the individuat
authorized to construct the wett. For non-standard construction, the report must include an as-buitt drawing showing details
such as depth, casing, perforated zones, and a description of the grouting type and intervat.
Pursuant to Rule 6.2,3 of the Water Well Construction Rutes, the wett construction contractor shatt submit the as-built wett
location on work reports required by Rute 17.1 within 60 days of completion of the wett. The measured locatìon must be
accurâte to 200 feet of the actuat location. The tocation information must include a GPS location (UTM coordinates) pursuant
to the Division of Water Resources' guìdelines.

Additionatty, pursuant to Rute 14.2 of the Water Wetl Construction Rutes (2 CCR 402-Zt, monitoring holes constructed pursuant
to a monitoring hole notice shatt not be converted to a production wett. (Upon obtaining a permit from the State Engineer, a
monitoring hole may be converted to a monitoring welt, recovery wetl for remediation of the aquifer, or a dewatering system
for dewatering the aquifer.)
NOTE: This permit witt expire on the expiration date untess the welt is constructed by that date. A Wett Construction and
Yietd Estimate Report (GW5-31) must be submitted to the Division of Water Resources to verify the wetl has been constructed.
