Our observations and recommendations are presented in thisreport. The services are supplemental to Kumar & Associates agreement for professionalservices to Z Construction dated February 15,2019.We previously designed the OWTS for the office building as submitted in our report dated April24,20l9,Project No. 19-7-143.02. The system was designed for four employees. The OWTSwas installed by Tonka Excavation.We performed our evaluations on August 26 and October 25,2019. The OWTS componentsobserved included: sewer line, cleanout, septic tank, effluent line, control panel and soiltreatment area (STA) and the locations of these components. The installation and locations ofthe system components were verified as shown on Figure 1. Pictures of the OV/TS componentsmade during the site visit are included with this report. The distance locations to the septic tankshown on Figure 1 were made by taping.On October 25,2019, after power had been supplied to the pump, a pump performance test wasperformed with adequate pump pressure observed.It is our opinion that the OWTS was installed in general conformance with the above referenceddesign.
