In accordance with and pursuant to the provisions of the Garfield County Land Use
and Development Code, as amended, and Resolution No. 93-109, the Director of
the Community Development Department hereby authorizes by amendment to the
Special Use Permit, the following activity:
TELECOMMUNICA TION FACILITY
The requested amendment is to remove 6 antennas, 3 RRHs and 1 OVP box with
cables and add 6 new antennas, 6 new radios, 6 diplexers, and 1 new OVP box
with cables.
The Amended Special Use Permit is issued subject to the conditions set forth in
Resolution 93-109, the Director's Decision dated February 1, 2013 (attached as
Exhibit 3), and this Director's Decision for a Minor Modification (Conditions
attached here as Exhibit 1), and shall be valid only during compliance with such
conditions and other applicable provisions of the Garfield County Land Use and
Development Code, as amended, Building Code, and other regulations of the
Board of County Commissioners of Garfield County, Colorado.

Conditions of Approval
1. All representations of the Applicant contained in the Application submittals
shall be conditions of approval unless specifically amended or modified by
the conditions contained herein.
2. All previous conditions of approval contained in Resolution 93-109 and the
Director's Decision Letter dated February 1, 2013, shall remain in full force
and effect.
3. The Applicant shall maintain compliance with all standards as required by
the Land Use and Development Code of 2013 and previous approvals.

RE: Director's Administrative Interpretation
Verizon Wireless Minor Modification Request
Dear Brad:
This Administrative Decision is provided in response to your request to consider
a minor modification of an existing Special Use Perrnil. The request is in regard
to the approved Telecommunication Tower for the U.S. West New Vector Group
an a site located al 310 County Road 138, in Section 35, f5S, R9OW, Parcel No.
2123-351-00-120. The request totter and associated plans are attached to this
Director's Determination.
As provided, your propuarll includes the installation of two additional omni -whip
antennas to the existing tower. The antennas wi#1 not increase the lower height
and will not result in the addition of any new structures on the site Additional
underground cables including fiber optics will be installed and connected to the
existing equiprnent shelter. New equipment will also be inslalled within the
existing equipment shelter but will not be visible from or result in any outside
changes In 1he shelter. The original tower was approved pursuant to a public
hearing conducted by the Board of County Commissioners and memorialized in
Resolution No. 93-109.
This amendment request is being reviewed in accordance with Section 4-106 of
the Unified Larxl Use Resolution of 2006, as amended and in accordance with
Condition No. 8 ai Resolution No. 93-109. The request has also heed reviewed
and found to be consistent with a Departmental Policy on co -location of
telecommunication facilities dated April 18, 2008 (see attached).
Director's Determination
Your installation of two additional omni -whip antennas and related support
equiprnent/cables below grade and connected to er within the existing equipment
shelter is approved as a Mirror Modification to ani approved Land Use Change
Permit (Special Use Permit) on your existing site and tower at 319 Courtly Road
