Subject: As -Built Evaluation of New Onsite Wastewater Treatment System, Proposed
Residence, Lot 3, Spring Ridge Place, Spring Ridge Court, Garfield County,
Colorado
Dear Lukasz:
A representative of H-P/Kumar observed the installation of the Onsite Wastewater Treatment
System (OWTS) for the residence under construction at the subject site as required by the
Garfield County Environmental Health Department. Our observations and recommendations are
presented in this report. The services are supplemental to our agreement for professional
services to Lukasz Lowisz dated June 7, 2018.
H-P/Kumar previously designed the OWTS for the residence as submitted in our report dated
July 31, 2018, Project No. 18-7-397.02. The system was designed for four bedrooms.
We performed our As -Built evaluation on December 12, 2018 and April 19, 2019. During the
site visits, the OWTS components observed included: sewer line, cleanouts, septic tank, effluent
line, distribution box and soil treatment area (STA) and the locations of these components. At
the time of the initial visit in December, the sewer line, cleanouts and septic tank had been
installed. The effluent line, distribution box and STA were installed at the time of the second
site visit in April. The installation and locations of the system components were verified as
shown on Figure 1, Record Drawing. The location of the tank was measured by taping from the
residence. Pictures of the OWTS components made during our site visits are also included with
this report.
ADS Arc 36 chambers were used instead of Infiltrator Quick 4 Plus Standard chambers. The
number of chambers was reduced to 12 chambers per trench and 48 chambers total from 15
chambers per trench and 60 chambers total due to the larger area of the Arc 36 chambers for a
total soil treatment area of 720 square feet (design size was 720 square feet). The distribution

box was changed to a Polylok 8 -hole distribution box from a Tuf-tite 6 -hole box. The noted
changes from our design were discussed prior to the installation are acceptable to us.
It is our opinion that the OWTS was installed in general conformance with the above referenced
design.
The observations and recommendations presented above are based on our site observations and
our experience in the area. We make no warranty either express or implied. Alterations to the
system or improper backfill and site grading could affect the operation and effectiveness of the
OWTS.
