CERTIFICATION OF TRUST
The undersigned Trustee, pursuant to Section 19-3B-1013 of the Code of Alabama (1975,
as amended), declare and state as follows:
1. Declarant, the currently acting Trustee of the Trust, certify the existence of the following
described Trust and facts regarding said Trust:
Name of Trust: The Thomas Andrew Minetree Revocable Trust
Made under the laws of the State of Alabama
Original Date of Execution of Trust: February 15, 2013
Name and address of the Trustee now qualified to act under the Trust instrument and who
is the only qualified Trustee: Thomas A. Minetree, PO Box 2408, Muscle
Shoals, Alabama 35662
Grantor: Thomas A. Minetree •
Successor Trustees: In the event that Thomas Andrew Minetree is unable to
serve as Trustee, or upon the death or disability of Thomas Andrew
Minetree, then, Malisa Minetree Hicks shall serve as Trustee. In the
event that neither Thomas Andrew Minetree nor Malisa Minetree
Hicks are able or wiling to serve as trustee, then Nicole Minetree Dill
shall serve as Trustee.
2. Declarant certifies that the Trustee, or all Trustees in the case of Co -Trustees, is required
to execute documents of conveyance transferring assets from the Trust.
3. Declarant certifies that the Trustee has all of the power and authority to act as set forth in
Section 19-3B-816 of the Code of Alabama (1975, as amended). Said Trustee actions
shall be taken by majority consent of all Co -Trustees serving as Trustee at such time in
the event of multiple trustees.
4. Declarant certifies that the Trust is in full force and effect and has not been revoked,
modified or otherwise amended in any manner which would cause the representation in
this Certification to be incorrect.
5. The Trust is revocable, and the sole person holding the power to revoke is Thomas
Andrew Minetree during his lifetime.
6. The manner in which title to Trust assets is to be taken is Thomas Andrew Minetree,
Trustee under the Thomas Andrew Minetree Revocable Trust originally dated
February 15, 2013.

7. Declarant certifies there are no claims, challenges of any kind or cause of action alleged,
contesting or questioning the validity of the Trust or the Trustee's authority for the Trust.
8. Declarant states that the Trust is governed by the laws of the State of Alabama.
This document is to be signed by the sole currently acting Trustee as of December
2018.
The undersigned declares, under penalties of perjury, that the statements contained herein
are true and correct and are made pursuant to the laws of the State of Alabama, and that this
