The undersigned, MARGARET TOLAN, being of lawful age and first duly sworn upon oath,
depose and state the following:
1. My name is MARGARET TOLAN, along with my husband SANTIAGO MUSSI
TISCORNIA, I purchased the property located at 352 Surrey St., Carbondale, CO 81623,
with the legal description of Section: 36 Township: 7 Range: 88 Subdivision: ELDER
AMENDED Lot: 6 25613 SQUARE FEET, evidenced by a Warranty Deed recorded at
Garfield County Clerk and Recorder's Office at Rec. No. 918116 ("Property").
2. After purchasing the Property, we built a residential Single -Unit Dwelling pursuant to the
2015 International Residential Code adopted by the Garfield County Board of County
Commissioners effective July 1, 2016.
3. I hereby affiiin that the Property will not include a secondary dwelling unit with lock -off
door and complete independent living facility.
4. We will therefore not apply for, nor is there a need for, an additional permit for a secondary
dwelling unit at this point in time.
