On July 15, 2020, we visited the site at 1188 County Road 106 near Carbondale, Colorado to
observe the OWTS installation pursuant to OWTS Permit # SEPT -6259. During our site visit, we observed
that the contractor had installed a cleanout between the house and septic tank, a precast concrete two-
compartment, 1,2500 -gallon septic tank with a pump in the second compartment of the tank, pump floats,
a pump control panel and alarm, effluent lines, and three pressurized effluent lines distributing effluent to
three feet of an unlined sand filter consisting of secondary sand filter media. The septic tank and manifold
were installed level, with risers installed on the septic tank to provide access to grade. Inspection ports
were installed at each corner of the sand filter. In our opinion, this OWTS was installed in general
conformance with Garfield County OWTS Regulations. No further action or review from Garfield County
Environmental Health is required regarding this permit. An As -Built drawing of the system is not required,
as the new system components were installed in the same locations as the replaced components.
