On July 6, 8, and 13, 2020, we visited the site at Lot 14 of Pinyon Peaks near Carbondale,
Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT -6169. During our various
site visits, we observed that the contractor had installed a cleanout between the house and septic tank, a
poly two-compartment, 1,000 -gallon septic tank, distribution box, and four gravelless chamber trenches
consisting of 11 Quick4 Infiltrator chambers per trench (44 total chambers). Proper encasement of the
effluent line from the septic tank to the distribution box was provided where the effluent line crosses the
potable water lines to the house. The septic tank and distribution box were installed level, with risers
installed to provide access to grade. Flow equalizers were installed in the distribution box, to distribute
effluent equally to the four trenches. An effluent filter was installed in the outlet tee of the septic tank.
Inspection ports were installed at each end of the trenches. Chambers were installed at an appropriate
depth and level. In our opinion, this OWTS was installed in general conformance with Garfield County
OWTS Regulations. No further action or review from Garfield County Environmental Health is required
regarding this permit. An As -Built drawing of the system is not required, as the new system components
were installed in the same locations as the replaced components.
