Changes to the design and permit must be approved by Garfield County Public Health and the designer prior to construction. The new OWTS will serve a portion of the Valley View Business Center and replace a failed existing seepage pit. The new OWTS will consist of a 1,500 -gallon, two-compartment pre -cast septic tank set level and accessible at grade with an effluent filter in the outlet tee of the tank. The tank lid will have an H-20 rating, as the tank will be in a parking lot. Effluent will gravity flow to the new seepage pit, which consists of an excavation of 14 -feet long by 14 -feet wide by 10 -feet deep (756 ft2)with a 4 - foot diameter structure in the center of the excavation consisting of 2 -foot tall perforated concrete rings, surrounded by %-inch diameter screened gravel. The structure will be placed on 6 -inches of gravel on the bottom of the excavation. Per Garfield County OWTS Regulations, "Schedule 40 or pipe of equivalent or greater strength must be used for the placement of piping under driveways or roadways and in instances where sewer line setback distances are granted a variance for any reason." (43.9.D.1.d.) All piping for this OWTS shall be Schedule 40 or stronger. Maintain all setbacks and elevations as designed. Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit, the applicant hereby agrees to comply with all laws and regulations related to the zoning, location, construction and erection of the proposed OWTS and associated structures for which this permit is granted. The applicant further agrees that if the above said regulations are not fully complied with in the zoning, location, erection and construction of the above described OWTS and structure, the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability, nor create presumption that the local public health agency or its employees may be liable for the failure or malfunctioning of any system. Permit issuance will not constitute a certification that the system, the equipment used in the system, or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit. Permit Approval Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from the date of issuance unless construction on the system has commenced.
