Lift Procedure
Contractor will attempt to lift foundation. Ability to lift foundation will be subject to field condtions and restoring foundation to original
position is not guaranteed. Customer is aware there is potential for cosmetic damage in the treated area(s). Contractor not liable
for cosmetic repairs caused by this process.
5 Year Annual Service Plan
Prepaid 5 year service plan to inspect and maintain products installed.
Re-Proofing/Pier Adjustments
Refer to customer notes section.
Push Pier 288
Individual Piers pushed to a depth greater that fifty feet (50') will incur additional cost at $30 per foot. Final results will not be known
until installation is completed.
Utility locates will be scheduled and completed before installation starts.
PolyRenewal
Install PolyRenewal to lift, level, and/or stabilize concrete slab.

*Customer has hired 3rd party engineer to create plan/make adjustment to contractors proposed plan. Contractor may hire 3rd party
engineer to create an engineered plarrandw€ll handle,permitting: *Contractor to provide install as recommended by engineer and will
not deviate from the engineered plan- this initial plan is not the finalized plan. If the engineer makes adjustments to the proposed plan,
the contractor will create an updated proposal in line with the engineered plan. This may result in the overall cost of the project fluxuating
up or down. Contractor does not claim to be a licensed engineer. Contractor will attempt to lift/level/stabilize structure, however unforeseeable
circumstances may limit the amount of lifting possible. Likely, damage will be caused due to lifting- contractor will not be held responsible
for damages caused by lifting procedure. Contractor to re -proof designated existing piers to attempt to level/stabilize existing pier areas
of structure, however if the piers have experienced complete failure, contractor may have to abandon the failed pier and install a new
pier. Customer to have utilities disconnected prior to installation. Customer to have finished flooring/finished walls/obstructions in work
areas removed prior to installation to provide access for crew. Contractor will remove concrete as necessary, however will not be responsible
for replacement. Contractor will attempt to lift/level/stabilize basement concrete utilizing PolyRenewal- however lift may be limited due
to the nature of the concrete. The concrete will be stabilized by the PolyRenewal. The PolyRenewal will be charged as an "allowance",
meaning the indicated amount on the proposal is based on the approximate Ibs needed to lift/stabilize the concrete. If the amount of
PolyRenewal is less than the estimated Ibs, or more than the estimated Ibs, the contractor will either credit or charge a fee of $10 per
Ib. Ultimately, the cost for the Poly will be $10 per Ib. Due to the nature of the area in which the structure is located, the contractor may

not be able to provide a full warranty on pierring on poly, however the contractor may come service the home and make adjustments/provide
additional product as necessary.

Groundworks Colorado, LLC d/b/a Foundation Repair of Western Colorado (the "Contractor").
