Cole III Subject: Final OWTS Inspection 2321 County Road 100 Carbondale, CO 81623 Final Acceptance Letter OWTS Permit #: SEPT-6140 September 18, 2020 Mr. Cole, Public Health 2014 Blake Avenue Glenwood Springs, CO 81601 (970) 945-6614 On April 10 and August 18, 2020, we visited the site at 2321 County Road 100 near Carbondale in Garfield County, Colorado to observe the Onsite Wastewater Treatment System (OWTS) installation pursuant to OWTS Permit # SEPT-6140. At the time of our April 10 site visit, the owner had installed the new sewer line and a 1,250-gallon two compartment concrete septic tank with a pump in the second chamber of the tank. Risers were installed on the access ports of the septic tank to provide access from grade, and the tank had been set level. A cleanout was installed between the house and the tank. At the time of our August 18 site visit, the owner had connected the distribution line from the pump to the Soil Treatment Area (STA). The STA consists of an unlined sand filter comprised of 24-inches of sand material. As per the design, three distribution laterals were installed to distribute effluent to the STA. The effluent line and distribution laterals consist of 1.5-inch Schedule 40 PVC. Orifice spacing was per the design report, and orifice shields were installed as specified. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Please provide an As -Built drawing of the system, when available, for our files. Edward R.
