Box 144 Snowmass, CO 81654 Subject: Final OWTS Inspection TBD Homestead Road Glenwood Springs, CO 81601 Final Acceptance Letter OWTS Permit #: SEPT-6292 September 25, 2020 Mr. Choma, 2014 Blake Avenue Glenwood Springs, CO 81601 (970) 945-6614 On September 4, 2020, we visited the proposed Choma Residence at TBD Homestead Road near Glenwood Springs in Garfield County, Colorado to observe the Onsite Wastewater Treatment System (OWTS) installation pursuant to OWTS Permit # SEPT-6292. At the time of our site visit, the contractor had installed the sewer line, a two compartment 1,500-gallon concrete septic tank with risers added to bring access to grade, effluent lines, a new distribution box with risers to bring access to grade, and six gravelless chamber trenches. The six gravelless chamber trenches consist of 15 chambers per trench (90 chambers total). An effluent filter was present on -site, but not installed, at the time of our site visit. The owner stated that the filter would be installed in the outlet tee of the tank. The trenches were slightly out of level in a few spots. The owner indicated the contractor would grade the trenches to be level. Finally, a cleanout was not installed between the house and the septic tank. The owner provided a photo showing the installation of the cleanout after our site visit, as well as the level graded trenches. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit.
