Per the conditions of approval listed in the Resolution 2010-08 associated with the GV 82-5 pad
location:
9. The Applicant shall be required to subnit a report annually, until such tine as the well pad
is released from the COGCC reclamation bond, of the extraction and processing operation,
for staff revieiv. Upon revieir of any, deficiencies pursuant to conditions of approval or other
local, state, or federal pernits, Staff nay forivard the report to the County Connissioners for
fidl revieiv of the Special Use Pernit.
On the GV 82-5 location, normal operations occurred this year which include the following:

Pre -emergent Treatment
There was no reclamation or worhover activity on this pad for 2020.
TEP personnel and contractors contintially monitor and maintain pi-odtictlon cgtllpment, water
removal, and production as needed.
TEP has reviewed the conditions set forth in the Major Impact Reviev,- and remains compliant under
the conditions of the resolution 11 sted above.
