You, the property owner, installed the system. The OWTS design is based on 3-bedrooms in an accessory dwelling unit (ADU). An average daily wastewater flow of 450 GPD was used. Use of the ADU near -term will include 2-bedrooms with a commercial kitchen used for baking cakes. We recommend monitoring wastewater flows by keeping track of pump cycles. The quality of the wastewater should not be significantly impacted by a cake baking operations. The sewer line exiting the residence has a double -sweep clean out and a minimum 2% grade to the septic tank. The system installation included a 1250-gallon, two -compartment concrete septic tank with an Orenco® Biotube Pump Vault and an Orenco® PF3005 pump in the second compartment of the septic tank. There is an effluent filter in this compartment. This filter must be cleaned annually, or as needed. Deadmen were used to anchor the septic tank due to high seasonal groundwater conditions. The floats were set to dose approximately 70 gallons each pump cycle, allowing approximately 4 gallons of drain back after each pump cycle. The control panel for the pump is located within line of sight of the septic tank. Valley Precast out of Buena Vista performed start-up of the pumping system. The excavation for the sand filter began 2-feet below native grade with a level base for the sand filter. The 1.5-inch diameter pump line maintained a minimum 1 % slope for drain back to the septic tank after each pump cycle. Effluent is pressure dosed through a 1.5-inch diameter pump line to a 12' x 47' partially mounded, unlined sand filter with a minimum of 2-feet of sand filter material. Laterals were connected with a 1.5-inch diameter level manifold. Laterals used to disperse the effluent were surrounded by washed coarse screened gravel. Laterals were 1.5-inches in diameter with 5/32-inch diameter orifices facing down, spaced 3-feet on center (OC). Each 1.5-inch diameter lateral ends in a sweeping ell facing up with a ball valve for flushing. A soil separation fabric was placed over the gravel layer followed by approximately 1-foot of topsoil or other suitable soil able to support vegetative growth. The mound has a minimum 3:1 slope (horizontal:vertical). The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Changes must be approved by Garfield County Public Health and the designer prior to construction. The new OWTS will serve a two -bedroom residence plus a commercial baking kitchen that may be converted to a bedroom in the future and consist of a 1,250-gallon, two -compartment concrete septic tank set level and accessible at grade with a pump in the second chamber of the tank, followed by a 1.5" diameter manifold set level to distribute effluent to 564 square feet of Soil Treatment Area (STA). The STA consists of 3 level 1.5-inch diameter laterals placed on a mounded, unlined sand filter with inspection ports at each corner of the bed. Orifice holes in the laterals are to be 5/32-inch diameter facing down, with orifice shields, drilled 3 feet on -center along the laterals. Maintain all setbacks and elevations as designed. Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit, the applicant hereby agrees to comply with all laws and regulations related to the zoning, location, construction and erection of the proposed OWTS and associated structures for which this permit is granted. The applicant further agrees that if the above said regulations are not fully complied with in the zoning, location, erection and construction of the above described OWTS and structure, the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability, nor create presumption that the local public health agency or Its employees may be liable for the failure or malfunctioning of any system. Permit issuance will not constitute a certification that the system, the equipment used in the system, or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit. Permit Approval Plans and specifications for the proposed OWTS have heen reviewed and are considered satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from the date of issuance unless construction on the system has commenced. Don't: • Pour cooking grease or oil down the sink or toilet. • Rinse coffee grounds into the sink. • Pour household chemicals down the sink or flush them. Do: • Eliminate or limit the use of a garbage disposal. • Properly dispose of coffee grounds a food. • Put grease in a container to harden before discarding in the trash. Don't: • Flush non -degradable products or chemicals, such as feminine hygiene products, condoms, dental floss, diapers, cigarette butts, cat litter, paper towels, pharmaceuticals. Do: • Dispose of these items in the trash can! Don't: • Park or drive on your drainfield. The weight can damage the drain lines. • Plant trees or shrubs too close to your drainfield, roots can grow into your system and clog it. Do: • Consult a septic service professional to advise you of the proper distance for planting trees and shrubs, depending on your septic tank location. Don't • Concentrate your water use by using your dishwasher, shower, washing machine, and toilet at the same time. All that extra water can really strain your septic system. Do: • Stagger the use of water -generating appliances. This can be helpful especially if your system has not been pumped in a long time. • Become more water efficient by fixing plumbing leaks and consider installing bathroom and kitchen faucet aerators and water -efficient products.
