Onsite Wastewater Treatment System (OWTS) Installation Observations

CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on July 7, 2020 for
the subject property. Avalanche Excavation installed the system.
The OWTS design is based on 3 -bedrooms. An average daily wastewater flow of 450 GPD was
used.
The 4 -inch diameter, SDR -35 sewer line exiting the residence has double -sweep clean out and a
minimum 2% grade to the septic tank.
The system installation included a 1000 -gallons, two compartment Roth poly septic tank with an Orenco®
Effluent Filter on the outlet tee. This filter must be cleaned annually, or as needed. Risers bring the
manhole lids to grade for access.
Effluent gravity flows to a distribution box, and then to four gravelless chamber trenches. The distribution
box is accessible from grade and has flow equalizers installed on each outlet pipe in the distribution box
to assure equal flow to each bed.
Each trench consists of 11 `Quick 4' Infiltrator® chambers for a total of 44 chambers and 528 square feet
of infiltrative area. There was at least 4 -feet of undisturbed soil between each trench. Inspection ports
were installed at the beginning and end of each trench. Ports may be cut to grade and placed in sprinkler
boxes for access.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.
LIMITS:
Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.

Permit Conditions:
This REVISED OWTS Permit is approved on the condition of compliance with the report and revised
design as submitted by CBO, INC. dated June 6, 2020. The previous OWTS Permit SEPT -6169 is null
and void. Changes to the revised design and permit must be approved by Garfield County Public
Health and the designer prior to construction.
The new OWTS will serve a three bedroom residence and consist of a 1,000 -gallon, two-
compartment pre -cast concrete (or poly) septic tank set level and accessible at grade with an
effluent filter in the outlet tee of the tank followed by a distribution box set level and to grade at the
to distribute effluent to 528 ft2 of Soil Treatment Area (STA). The STA consists of four gravelless
chamber trenches consisting of 11 Quick4 Infiltrator chambers per trench (44 chambers total).
Inspection ports are to be installed at each end of the trenches. Maintain all setbacks and elevations
as designed.
Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final
inspection of the installed system prior to backfill.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system, or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.

On July 6, 8, and 13, 2020, we visited the site at Lot 14 of Pinyon Peaks near Carbondale,
Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT -6169. During our various
site visits, we observed that the contractor had installed a cleanout between the house and septic tank, a
poly two-compartment, 1,000 -gallon septic tank, distribution box, and four gravelless chamber trenches
consisting of 11 Quick4 Infiltrator chambers per trench (44 total chambers). Proper encasement of the
effluent line from the septic tank to the distribution box was provided where the effluent line crosses the
potable water lines to the house. The septic tank and distribution box were installed level, with risers
installed to provide access to grade. Flow equalizers were installed in the distribution box, to distribute
effluent equally to the four trenches. An effluent filter was installed in the outlet tee of the septic tank.
Inspection ports were installed at each end of the trenches. Chambers were installed at an appropriate
depth and level. In our opinion, this OWTS was installed in general conformance with Garfield County
OWTS Regulations. No further action or review from Garfield County Environmental Health is required
regarding this permit. An As -Built drawing of the system is not required, as the new system components
were installed in the same locations as the replaced components.

• Have your system inspected (in general) every three years by a licensed contractor
and have the tank pumped, when necessary, generally every three to five years.
Don't:
• Pour cooking grease or oil down the
sink or toilet.
• Rinse coffee grounds into the sink.
• Pour household chemicals down the
sink or flush them.
Do:
• Eliminate or limit the use of a garbage
disposal.
• Properly dispose of coffee grounds a
food.
• Put grease in a container to harden
before discarding in the trash.
Don't:
• Flush non -degradable products or
chemicals, such as feminine hygiene
products, condoms, dental floss,
diapers, cigarette butts, cat litter, paper
towels, pharmaceuticals.
Do:
• Dispose of these items in the trash can!
Don't:
• Park or drive on your drainfield. The
weight can damage the drain lines.
• Plant trees or shrubs too close to your
drainfield, roots can grow into your
system and clog it.
Do:
• Consult a septic service professional to
advise you of the proper distance for
planting trees and shrubs, depending on
your septic tank location.
Don't
• Concentrate your water use by using
your dishwasher, shower, washing
machine, and toilet at the same time.
All that extra water can really strain
your septic system.
Do:
• Stagger the use of water -generating
appliances. This can be helpful
especially if your system has not been
pumped in a long time.
• Become more water efficient by fixing
plumbing leaks and consider installing
bathroom and kitchen faucet aerators
and water -efficient products.
