Author . This application for a Building Permit must be signed by the Owner of the property, described above, or
an authorized agent. If the signature below is not that of the Owner, a separate letter of authority, signed by the
Owner, must be provided with this Application.
Legal Access. A Building Permit cannot be issued without proof of legal and adequate access to the property for
purposes of inspections by the Building Division.
Other Permits. Multiple separate permits may be required: (1) State Electrical Permit, (2) County OWTS Permit,
(3) another permit required for use on the property identified above, e.g. State or County Highway/ Road Access or
a State Wastewater Discharge Permit.
Vold Permit. A Building Permit becomes null and void if the work authorized is not commenced within 180 days of
the date of issuance and if work is suspended or abandoned for a period of 180 days after commencement.
I hereby certify that I have read this Application and that the information contained above is true and correct. I
understand that the Building Division accepts the Application, along with the plans and specifications and other
data submitted by me or on my behalf (submittals), based upon my certification as to accuracy. Assuming
completeness of the submittals and approval of this Application, a Building Permit will be issued granting
permission to me, as Owner, to construct the structure(s) and facilities detailed on the submittals reviewed by the
Building Division. In consideration of the issuance of the Building Permit, I agree that I and my agents will comply
with provisions of any federal, state or local law regulating the work and the Garfield County Building Code, OWTS
regulations and applicable land use regulations (County Regulation(s)). I acknowledge that the Building Permit
may be suspended or revoked, upon notice from the County, if the location, construction or use of the structure(s)
and facility(ies), described above, are not in compliance with County Regulations) or any other applicable law.
I hereby grant permission to the Building Division to enter the property, described above, to inspect the work. I
further acknowledge that the issuance of the Building Permit does not prevent the Building Official from: (1)
requiring the correction of errors in the submittals, if any, discovered after issuance; or (2) stopping construction
or use of the structure(s) or facility(ies) if such is in violation of County Regulation(s) or any other applicable law.
Review of this Application, including submittals, and inspections of the work by the Building Division do not
constitute an acceptance of responsibility or liability by the County of errors, omissions or discrepancies. As the
Owner, I acknowledge that responsibility for compliance with federal, state and local laws and County Regulations
rest with me and my authorized agents, including without limitation my architect designer, engineer and/ or
builder.

NOTE: If any required information is missing, delays in issuing the permit are to be expected. If
determined by the Building Official that additional information is necessary to review the application
and/or plans for compliance, the application may be placed on hold until the required information is
provided. Work may not proceed without the issuance of a permit.
The Building Division will collect a Plan Review fee at time of application submittal_ The Permit Fee, as
well as any Septic or Road Impact fees will be collected when the permit is Issued_
The permit application must be signed by the owner or by a person shaving written authority from the
owner to act as their representative.

GreenBuild Solutions, LLC. hereby submits this proposal setting forth the specifications and costs for the
Work described herein for the above referenced job name. By signature hereon Owner hereby accepts
this proposal, which is made a part of that certain Agreement submitted herewith, which shall be executed
simultaneously herewith.
Description of the Work:
GenFlex 60 mil TPO orequivalent - Single Ply, Heat Welded Membrane:
1. Roof removal — Not required, TPO overlay. Inspect metal and substrate decking for any
deterioration or damage. Replacement of any substrate will be completed on a time and material
basis.
2. Taper and/or Underlayment —Install 2"xl6" EPS filler in all panel pans. Instal1518" OSB
plywood, 12 fasteners per sheet. Install Fesco Perlite ''/z" fiberboard on top of plywood decking.
3. Fire Barrier —Not required or specified.
4. Roof Flashing — Standard Kynar 500 24 gauge pre -finished metal, specified color. Fabricate and
install all standard ftashings.
S. Roof Covering — GenFlex 60 mil TPO single ply membrane — white, tan, or gray. Supply and
install TPO, mechanically fastened and heat welded per manufacturer's specifications.
Warranty: GreenBuild Solutions, LLC. expressly warranties its installation hereunder to be free from
defects in workmanship for a period of 5 years. Our obligation under this warranty shall be limited to any
necessary labor if, within the warranty period, any portion of our work shall be demonstrated to have been
improperly completed (or to contain defective materials for which the manufacturer has extended its
warranty as set forth further in the contract). If Owner shall make or permit, without prior written consent
from GreenBuild Solutions, LLC., repairs, alterations or additions to the roof which affect the Work, this
Warranty shall become null and void and of no further effect effective as of the date of commencement of
any such unauthorized repairs, alterations or additions. GreenBuild Solutions, LLC will respond
promptly and reasonably to any written request for consent for repairs, alterations or additions.
GreenBuild Solutions, LLC will, upon prompt written notice from Owner, perform the labor necessary to
correct such defects in workmanship. This warranty specifically excludes damage caused by third parties,
damage from ice damming, condensation, acts of God, or damage caused by other trades during or after
the completion of our work. Owner hereby acknowledges receipt of the ice damming information package
and is aware that this warranty shall be void in the event roof repair, maintenance or snow removal are not
performed by GreenBuild Solutions, LLC. THIS WARRANTY IS IN ADDITION TO THE
MANUFACTURERS WARRANTY FOR MATERIALS AND IS EXPRESSLY IN LIEU OF ALL
