Subject: Certification of Onsite Wastewater Treatment System, Proposed Residence, Lat 4,
Native Springs, 14 Native Springs Drive, Garfield County, Colorado
Dear Marcus and Silvia:
A representative of Kumar & Associates observed the installation of the Onsite Wastewater
Treatment System (OWTS) for the residence at the subject site as required by the Garfield
County Environmental Health Department. Our observations and recommendations are
presented in this report. The services are supplemental to Kumar & Associates agreement for
professional services to you dated September 13, 2019.
'We previously designed the OWTS for the residence as submitted in our report dated December
27, 2019, Project No. I 9-7-543.02. The system was designed for four bedrooms.
We performed our evaluations on June 29 and July 13, 2020. The OWTS components observed
included: sewer line, cleanout, septic tank, effluent line, distribution box and soil treatment area
(STA) and the locations of these components. The installation and locations of the system
components were verified as shown on the attached Figure 1 Record Drawing. Pictures of the
OWTS components made during the site visit are included with this report. The distance
locations to the septic tank shown on Figure I were made by taping.
The designed Tuf-tite 4-hole distribution box was changed to a precast concrete 8-hole
distribution box. The change is acceptable to us.
An additional profile pit at the design STA was not excavated as recommended in our design.
The soil exposed in the trench excavations for the STA consisted of Sandy Loam which was is
what we assumed for our design. Previous borings we drilled at the lot for foundation design of
the residence encountered groundwater at 10 and lOYz feet depth when drilled in late April 2016.
Based on this information and our experience on this lot and several nearby other lots at the
Native Springs subdivision, we do not believe an additional profile pit is needed.
It is our opinion that the OWTS was installed in general conformance with the above referenced
design.

our experience in the area. We make no waranty either express or implied. Alterations to the
system or improper backfill and site grading could affect the operation and effectiveness of the
OWTS.
