Final Acceptance Letter

Ms. Van Berlo,
On February 20, 2021, we visited your residence at 176 Daisy Drive near
Carbondale in Garfield County, Colorado to observe the septic tank replacement
pursuant to OWTS Permit # SEPT-6685. The existing concrete septic tank had been
removed, and the new 1,250-gallon two-compartment poly septic tank had been
installed in the same excavation as the historic tank. Per our observations, the tank was
installed level. Sewage lines from the tank to the Soil Treatment Area (STA) were
plumbed with two 45-degree bends. The contractor had installed risers on the tank to
bring access to grade and installed a new effluent filter in the outlet tee of the new tank.
In our opinion, this tank was installed in general conformance with Garfield County
OWTS Regulations. No further action or review from Garfield County Environmental
Health is required regarding this permit.
