AS -BUILT CERTIFICATE OF INSPECTION & ACCEPTANCE

The referenced onsite wastewater treatment system (OWTS) was inspected February 9, 2021. At
the time of inspection, the OWTS leachfield had been inspected by Garfield County and
backfilled prior to calling WestWater Engineering for a final OWTS inspection. Prior to
conducting the final inspection, all septic components except the Quick4 Plus Standard Infiltrator
trenches had been exposed by the OWTS installer. The onsite meeting with OWTS installer
provided insight in pipe length and configuration out of the distribution box to the 3 trenches.
On the basis of inspecting exposed OWTS component, discussion with OWTS installer, and
engineering sign -off from Garfield County, the OWTS is deemed to be installed in conformance
with October 2020 engineered design plans. Contact WestWater Engineering for any additional
information regarding engineered -design OWTS for referenced address.
