On February 10, 2021, we visited the site of the proposed Montanez Residence on
County Road 237 near Silt in Garfield County, Colorado to observe the Onsite Wastewater
Treatment System (OWTS) installation pursuant to OWTS Permit # SEPT-6603. At the time of
our site visit, the contractor had installed a 1,250-gallon two compartment poly septic tank, the
new sewer line, the effluent line, the distribution box, and four chamber trenches. The tank,
distribution box, and chambers were installed level, with access to the tank and distribution box
brought to grade. An effluent filter was installed in the outlet tee of the tank after our site visit
(confirmed via photographs provided by the contractor). The four chamber trenches consisted of
20 Arc36 chambers per trench, for.80 total chambers and a total STA of 1,200 ft2. In our opinion,
this system was installed in general conformance with Garfield County OWTS Regulations. No
further action or review from Garfield County Environmental Health is required regarding this
permit. Please provide an As=Built drawing of the system, when available, for our files.
