Permit Conditions:
This OWTS Permit ís approved on the condition of compliance w¡th the desígn as submitted by
Kenneth J. Brotsky, P.E. dated March 76,202A. Changes must be approved by Garfield County Public
Health and the designer prior to construction.
The OWTS will serve a three-bedroom residence and consist of a 1",250-gallon, two-compartment
septic tank set level and accessible at grade,.followed by a PVC manifold set level and accessible at
grade to distribute effluent to 768 square feet of Soil Treatment Area (STA). The STA consists of 4
level gravelless chamber trenches, with 1.6 lnfiltrator chambers each (64 total) w¡th inspection ports
at the end of each trench. Maintain all setbacks and elevations as designed.
Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final
inspection of the installed system prior to backfill.
