Michael,
On May 27, 2021, we visited the residence at 138 Flying Fish Road near Carbondale in
Garfield County, Colorado to observe the septic tank installation pursuant to OWTS Permit #
SEPT-6865. At the time of our site visit, the contractor had removed the failed septic tank and
installed the new 1,250-gallon septic tank in the same location as the historic tank. The septic
tank was installed level, with access to the tank brought to grade. In our opinion, this system
was installed in general conformance with Garfield County OWTS Regulations. No further action
or review from Garfield County Environmental Health is required regarding this permit.
