Dear Mr. Bwk,
As requested, structural steel special inspection was conducted per the IBC 2015
Edition - Chapter l7: Structural Tests and Special lnspection and Garfield Counry
requirements. I arrived on-site at l0:30 AM. I met with you to review project
requirements and areas ready for inspection. The steel erection company for this project
is Pioneer Steel Inc. (PSI) in New Castle, CO. The welding procedure specifications
(WPS) and the welder certification test reports (V/QTR) were previously reviewed, and
all rvere observed to be acceptable with project requirements. Copies of certifications are
on file with Mr. Aaron Lyles with PSl.
Visualwelding inspection was conducted from the Foundation Plan to the Roof
Framing Plan levels on the field welded beam to column fillet welded connections as
required on the approved engineering stamped drawings provided by Rakowski-Millard
Structural Design, LLC in Carbondale, CO.: Sheet S2.0 to Só.4 dated 7-28-20.lnspection
was conducted at grid lines A -L &. I - 9. Upon completion, I observed all fillet welds ar
all levels refereneed to be in compliance with project specifications and Lhe visual
inspection requirements as stated in Clause 6 of the AWS Dl .l : Structural Welding Code-
Steel:2015 Edition.
High strength bolt inspection was conducfed from the Foundation Plan to the Roof
Framing Plan levels on the %" ASTM 4325 High Strength beam to beam and beam to
column bolted connections as required on Sheet Sl.0: General Requirements -- Steel,
Paragraph 4.2. Inspection was conducted at grid lines A -L &,1 - 9. During inspection, I
observed a total of 3 each bolts were observed to be unacceptable due to loose or were
missing a hardened steel washer under the tuming surface. I brought this to your
altention, and you immediately had workers on site with PSI repairing each repair
location referenced. upon completion, all bolts inspecled were observed to be in

compliance wilh project specifications and the inspection requiremenls as sfated in
Section 9 of the Specification for Structural Joints Using A3ã5 or 4490 Bolts issued by
the Research Council on Structural Connections.
Ultrasonic shear wave welding inspection was conducted at the Main Level Floor
Framing Plan and the Roof Framing Plan on the Complete Joint Penet¡ation (CJp) groove
welded connections as required on Sheet 56.3: Details l0 & 15. Please refer io thå -
attached "Ultrasonic Weld Inspection" form for weld locations and test results.
