Dear Mr. Gonzalez,
As requested, structural steel special inspection was conducted per the IBC 2018
Edition - C'hapter l7: Structural Tests and Special Inspection and the Garfield County
requirements. I arrived on-site at 8:30 AM. I met with you to review project requirements
and material ready for inspection. The steel fabricator and erector for this project is
Garfield Steel and Machine, Inc. (GSMI) in Rifle, Colorado. Prior to our arrival. we
reviewed copies of the welding procedure specifications (WPS) and the welder
certification test repCIrts (WQTR) and all were observed to be acceptable with project
requirements. Copies of reports are on file with Mr. I,yle Inman with GSMI.
Visual welding inspection was conducted on all levels of the residence. I observed
the construction clip to beam fìllet welds, glue-lam bucket to beam fillet welds, and the
shop welded fillet welds. Upon completion, I observed all fillet welds to be in compliance
with project specifications and the inspection criteria as stated in Clause 6 of the AWS
Dl.l Welding Code - Steel: 201 5 Edition.
High strength bolt inspection was conducted on all levels of the residence. I
observed all bolts tobe 3/q" & %" AS'IM 4325 plain body bolts with a hardened flat
washer properly positioned under the turning surface of a heavy hex nut. All bolts were
checked with a w¡ench for hand tight (HT) conditions as required per project
specifications. During my inspection, I observed 2 bolts that were too short to fully
engage the nut threads as required. These were replaced immediately by you on site.
Upon completion, I observed all bolts to be in compliance with project specifications and
the inspection criteria of the American Institute of Steel Construction (AISC) Research
Council on Bolted Connections (RCBC).

To Whom It May Concern:
I inspected the rebar layout for the cast in place concrete walls for the project at the above
address on Tuesday October 6th. With a few minor exceptions that were addressed in the
field, the forms and rebar were checked, and found to be, in general compliance with the
plans issued by my office dated September 21,2020.
The exceptions were
L An additional #5 longitudinal bar was put in the bottom of the wall on Grid 4
because of a large boulder conflicting with the reinforcement on the south west
corner.
2. The corner reinforcement at the North West and East corners was not installed.
Reference Detail B/S2.0.
Site visits, field observations and framing and foundation inspections made by Studio M
Engineers, LLC whether included in the basic scope of service or not, do not make Studio
M Engineers, LLC responsible for warranting or approving (expressed or implied),
directing, or superintending the contractor's work, means or methods of work, safety in,
on or near the work site, timeliness in performance of the work, nor any other aspect of
construction for which the contractor has responsibility. Site visits are for project
administration, assessment, observation or testing will not relieve the contractor of its
responsibilities for performing the work in accordance with the project plans and
specifications.
The final foundation inspections and approval sign-off shall be performed by the
Governing Building Department, without exception.

Re: Open Hole lnspection, Lot37, Antlers Orchard
Capstone enterprises West, LLC (Capstone) inspected the foundation excavation at Lot 37 in the Antlers
Orchard Subdivision. The base of the excavation was sandstone bedrock. These materials were as
anticipated form the original geotechnical report performed by Capstone.
Capstone recommends that the excavation be cleared of any loose rock, to prevent rock pockets in the
concrete footer. The concrete can be placed directly on the cleaned sandstone surface.
This bedrock is suitable for foundations designed with a maximum bearing load of 4,000 psf.
