CERTIFICATE OF INSPECTION AND ACCEPTANCE
On-site Wastewater Treatment System

The on-site wastewater treatment system of reference was inspected on October 12th 2021. The
system was found to conform to our plans and specifications set forth in the design for the
aforementioned property prepared May 28th, 2021. Copies of the engineered plan are on file at the
Garfield County Environmental Health Department or may be obtained from the undersigned
design engineer.
