Ogden Street Englewood, CO 80113 Via email: edward.vandenberg@comcast.com Subject: Final Acceptance Letter 284 Sun King Drive Glenwood Springs, CO 81601 OWTS Permit #: SEPT-6959 December 9, 2021 Edward and Marcia, On September 20, 2021, we visited the site at 284 Sun King Drive near Glenwood Springs in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT-6959. At the time of our site visit, the contractor had installed the new sewer line and cleanout adjacent to the residence, the new poly 1,787-gallon two-compartment septic tank, the new effluent line, the distribution box, and the seven chamber trenches. The septic tank and distribution box were installed level with access brought to grade. An effluent filter had been installed in the outlet tee of the septic tank. The chamber trenches were level, with 4-feet between the trenches. Each trench consisted of 18 ARC 36 chambers per trench for 126 total chambers (1,890 ft2). Additionally, inspection ports were present at each end of each trench. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Please provide an As-Built drawing of the system, when available, for our files.
